There are many cities, but the most inhabited is the one where everyone is currently located; which is what was once London, essentially. Many of the noticeable monuments remain (the London Eye, Big Ben, others) but in very poor condition. In the hold and in the city, things are dilapidated but they're still somewhat clean -- though as a character continues outward toward the other cities, it becomes more and more destroyed/abandoned/similar to photography of what is left now from the Chernobyl disaster. At the borders of the island are troops of the Masked, but they don't go out of their way to actually police beyond keeping watch unless absolutely necessary.
PCs are able to leave Exsilium! They can get past the guards, but outside of Exsilium -- (the United Earth, as it were) -- is much, much more strict (policed heavily by the Masked, constantly monitored, everyone wears the same clothes, everything feels colorless/clean in an unsettling way). Think part Equilibrium, part 1984, part Gattaca (if those references are helpful/relevant :( ). [ Brette adds that it is similar to Panem from The Hunger Games. ]
