The Initiative aspires to collect full documentation on the Transports when they are brought in -- not just medical records, but also work history, education history, and anything else that you would expect to come up in a background check on someone. However, this technology is still in development, so you can expect plenty of errors. Some characters may end up with their full medical records on file. Others might not have anything on record at all. Some may even have incomplete records, or the information may have gotten scrambled along the way and possibly even mixed up with someone else's.
Due to the currently untrustworthy nature of their records, the Initiative encourages all Transports to go through a basic medical check-up shortly after their arrival, and one would expect to see various propaganda posters to that nature throughout the Initiative hold. The check-up is not mandatory, though they may try to change that in the future.
In short, the status of your character's medical records is up to your discretion. If your character has never sought out medical care in Exsilium before, they could find their records completely intact, missing key information or missing entirely, or even be wildly inaccurate.
